Welcome to the new Colorado Medical Society Member Benefit
Plan. The plan is comprised of 5
components:
1.

guaranteed-issue medical insurance plans underwritten by United Healthcare; 3
traditional PPO plans with office visit co-pays and 5 High Deductible Health
Plans which are compatible with a Health Savings Account (HSA)
2.
HSA's administered by United Healthcare
3.
Voluntary
life and long-term disability insurance program created for your staff;
physicians are welcome to enroll.
For example, if you could offer a package of:
Life $25,000
AD&D $75,000
LTD $1,000/month to age 65
All for only $130 per year for employees under age 40, why wouldn't you? Only three
employees, or 35% of those eligible in your practice need to enroll for
coverage to be effective. This
program is designed to be offered at NO COST to your practice.
4.
A
Premium-only cafeteria plan allowing your employees to pay their medical
insurance premiums on a before-tax basis.
The required Plan Document to establish the Premium-Only Plan is
provided by Denver Reserve Corporation.
5.
Long-term care insurance COPIC Financial secured a discount for CMS members from several of the nation's leading long-term care insurance companies.
The
applications and enrollment forms necessary to enroll in the medical, life and
disability insurance plans and the cafeteria plan can be downloaded directly
from this web site. The medical
insurance enrollment forms need to be submitted to COPIC Financial before the
15th of the month prior to your desired effective date.
