H.R. 1 is expected to significantly reshape health care financing by shifting more funding responsibility from the federal government to the states—an adjustment that may strain already tight state budgets. Colorado Medical Society President Brigitta Robinson, MD, and Chief Strategy Officer Chet Seward will guide physicians through what this means for health care delivery and payment in Colorado, including a timeline for when key provisions take effect.


Learning Objectives:

1. Analyze the systemic impact of HR-1 legislation on Colorado’s health care coverage landscape.

  • Describe how federal changes in HR-1 interact with Colorado’s interlocking coverage systems (HIAE, hospital provider fee, reinsurance program)
  • Explain the timeline and cascading effects of policy changes from 2025-2032 on state budget sustainability

2. Assess the anticipated effects of Medicaid work requirements and ACA enhanced premium tax credit expiration on patient populations and access to care.

  • Identify populations at highest risk for coverage loss, including the estimated 100,000 individuals expected to lose Medicaid expansion coverage and 100,000 Coloradans who will may lose their commercial insurance due to 170% average premium increases
  • Evaluate lessons learned from Colorado’s 2023 PHE unwinding that resulted in 373,000 erroneous eligibility terminations
  • Describe the combined impact on Colorado’s individual insurance market, including effects on risk pools, reinsurance programs, and commercial coverage stability

3. Recognize the operational and workforce impacts of increased administrative burden on clinical practice and implement preparedness strategies to mitigate HR-1 impacts.

  • Anticipate workflow changes related to eligibility verification, prior authorization, and patient enrollment support
  • Identify strategies to prepare clinical and administrative teams for increased documentation requirements
  • Develop plans for managing anticipated increases in uncompensated care and strengthening eligibility tracking workflows to reduce patient churn
  • Engage in advocacy efforts using patient stories and local data
